Murray-Plains Health Spa requires a certain amount of information from you, if you’re a new client.  

• Massage Form – This form is completed at the time of your appointment.  
• The online Naturopathy Form (below) will need to be completed prior to your first Naturopathy consultation.
• The online Hypnotherapy form (below) will need to be completed prior to your first Hypnotherapy consultation. 
• Please note:  The Psychotherapy Questionnaire is a specialized form that will only need to be completed, if requested.
